It actually makes perfect sense. Back in the days of Ms-Dos sound cards used drivers to load and a program to adjust the volume of the various outputs of the card in Dos, which worked for each game but also could be adjusted additionally in games that supported their own volume adjustment, which often resulted in a very complex chaos of volume settings needed to be adjusted.
Now I guess DosBox has a hidden mixer volume setting that's ran when the program start, virtually emulating "a soundcard" for compatibility reasons with games. Which is causing Legend of Kyrandia to produce the quiet sound.

One additional important note - if you're using the Roland MT-32 music setting, without having an actual MT-32 hardware module being attached, or without a special emulation program running, the music will not sound like real MT-32 music was meant to sound at all. It would at best sound a bit different from a General MIDI setting and probably a bit messed up and strange at times, since DosBox doesn't emulate MT-32 by itself. Same goes for Roland Sound Canvas setting (supported in Legend of Kyrandia 2: Hand of Fate).
