It looks like a sophisticated version of HighGrow.....I really have no idea, maybe a clue?

It may a bit a bit obscure. However game was based on rather well-known pen-and-paper RPG. It slightly resembles a bit older 'Wasteland'.

That clown IS distinctive. Beatiful game and shocking (att the time it came out). It is Sanitarium.

Here's one for ya   (Note the lovely lighting glitch on the upper part of the walls. Old games really hate my PC  )
