The PC CD-ROM release of Hexen: Beyond Heretic, as well as its PlayStation and Sega Saturn ports, features CD-quality Redbook audio music and an animated CGI intro with background narration. The Sega Saturn port is also said to have several new music tracks.

Also, here are videos of Powerslave on DOS and PlayStation. Not only the levels and the map are different, but also the advantages of Lobotomy's SlaveDriver engine are obvious compared to the early version of the Build engine that was used for the DOS version.

Duke Nukem 64, in addition to different sprites for weapons, enemies and such, as well as a lot of level changes mostly done due to censorship issues, also has a slide-show intro with captions, and a noticeably different version of the main theme (Grabbag) playing in the background.

I've worked on Mac port of World of Xeen, Mac port of Hexen: Beyond Heretic (also as the only engineer on the project...did most of the work in one weekend), Mac version of Lode Runner: The Legend Returns, Mac version of Lode Runner 2, and bits and bobs and technical proofs-of-concept at that company. Like, worked on a Mac port of Quake as part of contract negotiation...we didn't get that contract so I don't count it. :P

Beyond that, some PS1 and Dreamcast games, most notably ArmyMen 3D, NFL 2k, NFL 2k1, NBA 2k, and NBA 2k1.

Fallout
Being a German gamer, MrFlibble can speak volumes about changes in local game releases due to censorship (green blood anyone?), but what really put me off from the UK version of the game is the lack of intro song Maybe. Sadly this was also true in the case of bonus discs (full F1 & F2!) for Polish release of Fallout Tactics.

Imperialism 2
Is my memory failing me or the various releases (US/UK) have different musical score? ("exotic" versus classical chamber music)

Actually, I'm not from Germany, it's just that when I was registering here it was compulsory to indicate your location, but Russia wasn't on the list yet, so I picked Germany for no other reason than that I've visited the country a couple of times. Since then, I've been too lazy to edit the profile, so sorry if that was sort of misleading

But yeah, the censorship of video games in Germany, and those BPjS/BPjM indexes are pretty notorious.

Sometimes some content in the games seems to get "lost" in re-releases. Not long ago I got the Platinum Edition of Age of Empires (which includes both AoE and AoK and their respective expansions on a single DVD), and even though the tracks from the Collector's Edition Music CD are included (only as WAV tracks with no information though, even the titles), AoE and RoR only have MIDI music, and Aok and The Conquerors don't have even that (except the main menu, win and lose themes in MP3), so I had to copy MIDI tunes from the trial versions of the games and launch AoK with a command line parameter that forces MIDI music playback.

Apparently, there was a Special Edition of Tyrian that was bundled with some Acer products. This version, contains the first episode and the first half of the second one, effectively offering one half of the full game (originally consisting of three episodes). Unlike the shareware version, this one was not eligible to be freely distributed. It also seems to be based on a pre-release build of the game, and there are some noticeable differences from other versions, like different fonts and available weapons, and speech notifications appear to be missing too.

Interplay's classic Battle Chess had a CD re-release called the Multimedia PC version (or Battle Chess Enhanced CD ROM) with upgraded high-res 256-colour graphics.

For some reason the background story of Abuse had been changed entirely between versions 1.05 and 2.0. In v1.05, the main character is a covert operative of the human resistance under an oppressive alien rule, which was changed to a prison setting with mutated inmates as enemies, and no aliens at all, in v2.0. You can get the v1.05 shareware here.

I also have forgotten to mention that difference between the PC releases of Prince of Persia. In v1.0, the walls in the dungeon levels are everywhere of the same colour, but in v1.3 and v1.4 the dungeon walls are of standard colour in levels 1 and 2, but green in level 3 and brownish in later levels. The palace walls have varying tints of colour throughout the levels as well. This change seems to serve no purpose other than superficial variety, but personally I prefer the original colours from v1.0.

Similarly, the textures of the planet's surface in the first three levels of the first episode of Terminal Velocity had been changed from snowy white to grayish green between versions 1.1 and 1.2. Apparently, this has been done for aesthetic purposes, as the completely white snowy landscape looked too bland. You can get the v1.1 shareware here.

For a more recent example, Warcraft III: Reign of Chaos not only got a major overhaul of balance throughout its version history, but also some cosmetic changes as well. The Elven Priest and Lich icons have been changed (for no apparent reason), some different icons have been added that weren't in the initial release (like the neutral "magic immunity" icon), and I think some textures were updated as well. Truth be told, many of these changes retroactively introduced the additions from The Frozen Throne add-on for multiplayer compatibility and balance purposes, but some of them are really more aesthetic than functional. Maybe this is rather common with modern games, but the sum of changes throughout the updated of Warcraft III really makes a considerable difference if you compare v1.0 and the latest patched version.
