It happened with any disk, including known good floppies.
The message appeared on the first screen the computer showed upon being turned on. The computer had been stored in a closet, and was very gently treated. The floppy drive we replaced it with came from a computer stored in our garage, which gets very cold sometimes. Seems like the second one should fail, and the other should be the one that works......
