Well, when I first "drew up" the plans for this, I invisioned it being called "Medical School", and having you basically learn how to perform operations, or diagnose various conditions, then do a "quiz" to get your M.D. Once you have that, you can either train for other specialities or go into a hospital as a General Physician, and you have to diagnose patients and stuff, and you can go back for more training to further your studies, and as you sucessfully complete each degree/speciality it opens up more possibilites for diagnosing patients and performing operations. As in the original L&D games the focus would be on realism, and I would use real [up to a point] MRI, CAT, and X-RAY images to make it more realistic and such. Research and programming isn't a problem, everything would be easy with the exception of the OR, which would probably be the hardest part to do...

Can't help you there, I'm afraid, but you might want to specify what sort of graphics you've got in mind, perhaps... Or you could have a look at people's art threads and ask them directly if you like the sort of stuff they're doing.
