This worked for me.

DOSBox conf

serial1=modem

Modem configuration in the game options

modem type -> custom
COM port -> COM1 (serial1=COM1, serial2=COM2, etc...)
dial string -> ATD
init string -> ATZ

Use the IP address of the host as a phone number.
